a link to the online reporting form. Please take a moment to complete a brief survey about the III (and
NextGen) conference programming. If you attended the Conference, please click here to complete the online
CLE-CPE reporting form. Your entries on the forms are what we will use to report your attendance, so completing these forms
are essential to receive credit. Once you complete the online form you may download the documents below as needed for your
records.
